Staff Profile Request Form

Use of this form will better assist the Information Technology Services Division in meeting auditor recommendations on network authentication and security controls. Please complete the following "Online Form" if changes to personnel occur that will directly impact any of the following

  • Telephone system, voicemail system, including extension relocations, mailbox changes, etc.;
  • Computer equipment allocation, relocation, set-up, or modification;
  • LAN, VPN access, network security and related services including access requirements to specific network applications or resources including, but not limited to: GIS, CentrServe ClassInfo, FireHouse, FMW, GEMS, E-mail, MAS, etc.; or,
  • Civic Centre security system and keyless entry system, or photo ID card requirements.

Information Technology Services should be informed of any employee hiring, transfer, leave or termination that may have an impact on any system listed above. Information Technology Services should also be informed of all requests to relocate or modify existing services. Please submit Profile Requests well before the start or transfer date required.
